Overview
Over February and March, OPWDD is working to finalize the 2025-26 Community Transition Initiative for People with Developmental Disabilities. OPWDD will require the submission of a completed attestation from the participating providers. OPWDD must make payments no later than mid-March.

Attestations should be submitted as a signed PDF with “Community Transition” as the subject.
