Acting Commissioner Delaney Announces Federal Approval Authorizing Health Home Care Management Services

Dear Friends and Colleagues:

Today, New York took a major step forward in achieving People First Care Coordination in our system of services for people with developmental disabilities with the federal Centers for Medicare & Medicaid Services (CMS) approval of New York’s State Plan Amendment (SPA #17-0025)

The approval of the SPA will provide Care Coordination Organizations (CCOs) with the authority to provide Health Home Care Management services to people with developmental disabilities beginning July 1, 2018. 

This approval is an important step as we transition Medicaid Service Coordination and Plan of Care Support Services to Health Home Care Management and HCBS Basic Plan Support provided by CCOs.   

These changes reflect the requirements needed to offer the Medicaid Health Home Care Management service to individuals in New York State’s service system for people with developmental disabilities.

Health Home Care Management is an enhanced type of service coordination that will better support people, including those with complex needs. Under this service, a Life Plan will be created for each individual. The Life Plan will include information not only about the services they receive from OPWDD, but also physical, behavioral health and other healthcare and wellness services. This will result in more flexible and comprehensive service planning that will be better able to anticipate future needs.

We look forward to the successful launch of this improved service on July 1, 2018 in partnership with the new Care Coordination Organizations formed by experienced providers of developmental disability services across New York State.
