DSP Initial Evaluation
The DSP Initial Evaluation is to be completed by the supervisor, but can also be used as a self-evaluation by the DSP.  The Initial Evaluation includes the competencies that DSPs are expected to demonstrate within the first several months of employment.

DSP Annual Evaluation
The DSP Annual Evaluation is to be completed by the supervisor on an annual basis, but can also be used as a self-evaluation by the DSP.  The Annual Evaluation includes the competencies from the Initial Evaluation, as well as the additional competencies that DSPs are expected to demonstrate within the first year of employment.

DSP Evaluation By Self-Advocate, Family Member, or Other Advocate
The Evaluation for Self-Advocates, Family Members, or Other Advocates is to be completed by the self-advocate receiving supports, their family, or other advocates as desired.  The person completing this evaluation should have observed the DSP enough times to provide a fair assessment of the DSP’s skill.

DSP Competencies Evaluation Training for Supervisors
The DSP Competencies Evaluation Training for Supervisors is an available resource that agencies could use to educate their supervisors on the DSP Competencies, Evaluation Tools, and best practices for evaluation.  This training was created as a tool for agencies to introduce the new competencies and evaluation tools to their supervisors.
