Becoming a DISCO

Welcome to the Developmental Disabilities Individual Support and Care Coordination Organization (DISCO) Resource Page.  DISCOs are defined as specialized managed care organizations that provide comprehensive care planning and service delivery for individuals with developmental disabilities.

This resource page include responses to OPWDD’s call for Letter of Intent requesting parties who are interested in applying to become a DISCO or providing specialized developmental disabilities services through an existing managed care organization as well as presentation materials on the “How to Become a DISCO” video conference that took place on July 18, 2014.

Application for Certificate of Authority for DISCOs/Start-up Grant Application 

OPWDD released the Application for a Certificate of Authority for DISCOs/Start-up Grant Application on August 21, 2014.   This comprehensive document will be used to qualify organizations that have met the criteria for becoming a DISCO and have applied to participate in the program.  Please note that this document also included criteria that may be used to award DISCO managed care start-up grants to eligible organizations as outlined in the Request for Grant Applications Managed Care Start-Up cost for DISCOs section below. The timetable for final approval of the applications and COA authorization is dependent on the approval by the Centers of Medicare and Medicaid Services (CMS) of both OPWDD’s new 1915 (b) Waiver and amendments to its 1915 (c) Waiver.  Finance section as outlined on page 60 [Finance (PDQ Section XVIII., pg.19)] Data Book and associated spreadsheets may be obtained by contacting OPWDD at peoplefirstwaiver@opwdd.ny.gov.

Request for Grant Applications Managed Care Start-Up Costs for DISCOs

Attachment 10 of the Application for Certificate of Authority for DISCOs/Start-up Grant Application includes a Request for Grant Applications Managed Care Start-Up Costs for DISCOs funding opportunity to support start-up costs for DISCOs to support the transition to managed care, which will ultimately improve care coordination for individuals with developmental disabilities, including both physical health and long-term care services.  This funding is made available through the federal Balancing Incentive Program (BIP).
