The New York State Office for People With Developmental Disabilities (OPWDD) is issuing the attached addenda to the Care Coordination Organization / Health Home (CCO/HH) Program Evaluation Request for Proposals (RFP). The official CCO RFP Responses to Questions and a revised version of the RFP are attached here with the SUMMARY of RFP Version 2 Revisions – ADDENDUM 1. The newly issued Version 2 of the RFP contains substantive changes, which are explained in the Summary document. Applicants are advised to review the new version  of the RFA in its entirety.

Announcement of a Request for Proposals (RFP) for Care Coordination Program evaluation

OPWDD is pleased to announce the release of a Request for Proposals (RFP) for a comprehensive evaluation of its Care Coordination program.

This competitive opportunity will identify an experienced and qualified consultant to evaluate the effectiveness of care management services delivered through OPWDD’s seven Care Coordination Organizations.

The review will continue and build upon the work of OPWDD’s 2020 Stakeholder Workgroup on Improving the Life Plan and Person-Centered Planning Process. The evaluation will examine several important aspects of the CCO program such as its impact on overall health and wellness of the people being served, how well the program is meeting the needs of people from diverse and/or complex backgrounds, how the program interacts with service systems beyond the developmental disabilities system and the effectiveness of its health information technology. It will also identify any systemic challenges that are impacting the program’s effectiveness. 

The results of the program evaluation will help OPWDD understand how it can improve the quality and effectiveness of care coordination and the outcomes for people who receive care management services.

This initiative is part of a greater effort to enhance, improve and transform key aspects of the OPWDD service system as described in OPWDD’s 2023-2027 Strategic Plan. The Care Coordination program evaluation is supported by OPWDD’s COVID-19 relief funds awarded by the federal Centers for Medicare & Medicaid Services.
