Registered Provider List

We are revising the procedures for approval of registered providers. These revisions are being made in response to changes in New York State Law related to background check and incident management requirements.  

OPWDD has issued a revised version of the OPWDD Form 108: Registered Provider Approval Request Form, including a Registered Provider Approval Request Form Continuation Page. 

All current registered providers must complete the new form and submit it to OPWDD in order to retain approved Registered Provider status. 

All new requests for approval must be submitted on the new form.

Pursuant to 14NYCRR 633.22(o)(1), agencies providing certified and HCBS waiver services may contract with certain providers of service only if that provider is an authorized provider. The following list includes those providers that have submitted an application for registration and have agreed to certain terms as established by OPWDD as a condition of registration.

The entities listed below are considered to be "registered providers" by OPWDD. However, it should be noted that inclusion on this list does not constitute any position or opinion on the part of OPWDD, the State of New York or of its officers, employees, agents or representatives as to the ability of any of the entities on the list to provide any particular service or services.

The engagement and/or utilization of any entity on the list is the sole responsibility of the party engaging and/or utilizing the entity and in no event shall OPWDD, the State of New York or its officers, employees, agents or representatives be responsible for the performance or actions (or lack or manner thereof) by an entity so selected by any such party.
