Care Coordination Organizations - Information for Care Managers


Health Home Care Management is a new, enhanced way of providing care coordination for New Yorkers with developmental disabilities. Health Home Care Management is replacing Medicaid Service Coordination and will be offered by  Care Coordination Organizations that were formed by existing developmental disability service providers.

The important role of Medicaid Service Coordinators will continue within Care Coordination Organizations as you transition to expanded roles as Care Managers.  This new role combines valued MSC experience in developmental disabilities with new training on the coordination of services from other systems, such as medical, wellness, mental health and community supports.

The resources here, which include an MSC Toolkit, the MSC Resource Guide, webinars,  FAQs for MSCs and FAQs on Care Coordination and Consent,  are designed to assist you in your transition to your new role as Care Managers, including the critical role that you have today in enrolling individuals into Care Coordination Organizations by July 1, 2018.
