School Transition

Finishing high school and starting life as an adult is an exciting time for any student, including students with a developmental disability. Your school can help get you the information to start planning for your future. 

Planning makes the transition easier but you must apply early so the services are available when you need them. Your school will help you make a transition plan, which should be done by the age of 15, as part of your Individualized Educational Plan (IEP). If you plan ahead, your plan will already be in place when you turn 21 or graduate high school. You will make decisions on what you want to do after high school, with the assistance of people you choose, such as your family, friends, school counselor or others.

Your options include continuing with schooling, learning job skills, finding paid or volunteer work and participating in community activities. OPWDD can help you develop job skills, improve living skills, be more independent and be active in your community.

Parents/Guardians need to fill-out a consent form in order to get the transition planning started. Once that is completed, OPWDD will be authorized to work with school staff to begin the student's transition planning process.
