Active Representation

Representation is first and foremost for a Willowbrook class member.

Active Representation refers to a family member or advocate taking part in the planning and evaluation a person's plan of services. It is available to all Willowbrook class members.

It does not mean merely signing consent forms sent through the mail or receiving phone calls initiated by staff with no other involvement.

It means as the name implies - active involvement.

There are several ways Willowbrook class members can be represented. A class member can advocate for themselves, or they can have a co-representative such as a family member, advocate or the Consumer Advisory Board (CAB).  They can also choose to have full representation from a family member, advocate or the CAB.

The Consumer Advisory Board (CAB) is a seven-member board that provides necessary and appropriate representation and advocacy services on an individual basis for all Willowbrook class members as long as any class member lives. 

Whatever the choice of representation, notification is required.  Below are template letters that can be used for notification, depending on the type of representation.

Template letter to be used when advising a family member/advocate of the availability of the Consumer Advisory Board (CAB) on behalf of Willowbrook class members who are fully represented by family.

Template letter to be used when advising a family member/advocate of the availability of full representation from the Consumer Advisory Board (CAB) for Willowbrook class members who already have co-representation.

Template letter to be used when contacting the Consumer Advisory Board (CAB) to request full representation on behalf of a Willowbrook class member when there has not been a response from the family member/advocate.

Template letter to be used when contacting the Consumer Advisory Board (CAB) to request full representation on behalf of a Willowbrook class member.

Template letter to be used when contacting the Consumer Advisory Board (CAB) to request co-representation on behalf of a Willowbrook class member.
